Fees

We are In-Network for the following insurance plans:

- Blue Cross Blue Shield PPO
- Aetna
- Cigna
- United HealthCare
- Humana (in-process, anticipated March 2010)
- Coventry (in-process, anticipated March 2010)

Please note that we do not participate with Medicare, Medicaid or BCBS HMO at this time.  We welcome patients with these and other insurances on a self-pay basis.

Fees for patients paying directly for their care are as follows:

Typical new patient visits: $120
Typical follow-up visits: $80

Ponce Primary Care patients also have the option of receiving the many benefits of becoming a Member of the practice:

Level 1 Membership:  Direct access to Dr. Costley's cell phone, reserved same day or next business day appointments, phone visits, discounted visit and lab fees, 25% discount on Nutrition/Fitness consultations with David Orozco.

Quarterly Fee: $180  Initial Visit: $120   Established Patient Visit: $20   Labs: see Fee Schedule

Level 2 Membership:  All benefits of Level 1 Membership plus free office visits, $200 of labs per year included, 3 nutrition/exercise consultations with David Orozco and 3 phone consultations with Dr. Costley per year.

Quarterly Fee: $360  Initial Visit: $120   Established Patient Visit: $0   Labs: first $200 included
