Dr. Chad Costley's Bio

Dr. Costley is the founder and lead physician of Ponce Primary Care.  He is board-certified in Family Medicine, a diplomat of the American College of Preventive Medicine, and a member of the American Academy of Family Physicians.

Dr. Costley earned his M.D. degree from the University of Michigan where he also completed his undergraduate studies in English Literature.  Following medical school, he remained at the University of Michigan to complete his residency training in Family Medicine.  He has practiced medicine for over ten years in a variety of settings including teaching hospitals, urgent care centers, and outpatient clinics serving the poor and uninsured.  While his background includes significant pediatric and obstetric experience, Dr. Chad now concentrates his practice on providing exceptional, comprehensive and preventive care to teenage and adult patients.

In addition to his medical background, Dr. Costley holds a MBA degree from Emory University and serves on the board of directors of three technology start-up companies.

Dr. Costley is married to LeighAnn Costley whom he met at the University of Michigan.  They have two young children.  He enjoys distance running, tennis, and cooking.
